Greater Cleveland Community Health Checkup

Better Health Greater Cleveland's first of a series of reports, designed to help patients and providers achieve better health in the setting of chronic disease. The first Community Health Checkup focuses on diabetes, evaluating pratice-centered and patient-centered benchmarks for almost 25,000 patients in 2007. Comparisons are provided by race, income, educational attainment and insurance type. Practice-level quality variance is addressed, as are comparisons between our region and the nation.
